Step #1 - Paperwork
The 1st step to getting started with your DUI classes and meeting your licensure reinstatement requirements is to complete the online portion (paperwork) of your assessment. This link can be found at the top left of the website or down below. This portion of the assessment forms helps our assessor understand your situation and assists us in determining the best possible options for your individual needs during your in-person assessment. Your information is completely confidential.
Once you've completed the paperwork, go to step 2 to schedule an appointment for your in-person assessment.
If you cannot complete the assessment forms online and would prefer to complete on paper, please schedule an appointment by texting 859.229.3329.
Step #2 - Schedule Assessment
The 2nd step to enroll in DUI classes and meeting your licensure reinstatement requirements is to schedule an appointment to complete the in-person portion of your assessment with our assessor. By the end of the assessment, you will have a clear understanding of your how many classes you will need to attend and expectations as well as a fee schedule.
Lexington - Assessments - Sundays and Thursdays
Richmond - Assessments - Mondays
Winchester - Assessments - Saturdays
Other days and times may be available upon request, depending on the assessor's schedule. Please text us to help you find a time that is suitable.
The in-person assessment will take 30-45 minutes to complete.
Please bring a copy of your AOC494 and Citation or request a copy of your court form AOC494 and copy of your Uniform Citation be sent to our assessor by contacting the court that is handling your DUI case. They can be mailed to us or sent to our confidential fax at 855.921.1840.
The assessment fee will also be due at that time of your assessment. We accept Cash, Venmo, CashApp, and Zelle or Money Order.
