how to get started in 3 easy steps
Step #1 - Paperwork
Step 1: Complete the online portion (paperwork) of your assessment. This link can be found at the top left of the website or down below. This portion of the assessment helps us understand your situation and assists us in determining the best possible options for your individual needs during your in-person assessment. Your information is completely confidential.
Once you've completed the paperwork, go to step 2 to request an appointment for your in-person assessment.
If you cannot complete the assessment enrollment forms online and would prefer to complete on paper, please request an appointment by texting 859.229.3329.
Step #2 - Schedule Assessment
Step 2: Enroll by requesting an appointment for your comprehensive substance use and mental health assessment. This is the in-person portion of your assessment. By the end of the assessment, you will have a clear understanding of your how many classes you will need to attend and expectations as well as a fee schedule.
Lexington - Assessments - Sundays and Thursdays
Richmond - Assessments - Mondays and Wednesdays
Winchester - Assessments - Saturdays
Other days and times may be available upon request, depending on the assessor's schedule. Please text us to help you find a time that is suitable.
The in-person assessment will take 45 min - 1 hour to complete.

What to bring with you to assessment:
1. Form AOC494 (Notice to Attend) Must be completed and filed by the court.
2. Uniform Citation (Officer's report of charges)
3. Optional, but very helpful: Plea Agreement Also must be completed and filed with the court.
*If you do not have these, please request a copy of each to be sent to us by contacting the court that is handling your DUI case. They can be mailed to us, emailed to info@duiprogramsky.org or sent to our confidential fax at 855.921.1840.
The assessment fee will also be due at that time of your assessment. We accept Cash, Venmo, CashApp, or Money Order. No checks, credit, or debit cards are excepted.
